SOCIAL WORKER PRN

Job Description

We are looking for a skilled Social Worker to join our team at LifeBridge Health on a PRN (as needed) basis. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various issues such as adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address common problems including adjustment to institutional living, financial concerns, family relationships, and discharge planning.
• Provide counseling related to acceptance of medical procedures and treatments, aging, powerlessness, and end-of-life issues.

Requirements

• Bachelor's degree in social work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
