SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling. This position requires a Bachelor's degree, 1-3 years of experience, and relevant certifications like LBSW/LMSW/LGPC.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments and offer counseling on various issues.
• Assist with discharge planning and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
