Job Description

We are looking for a dedicated PRN Social Worker to join the team at LifeBridge Health in Baltimore, MD. In this role, you will provide essential clinical social work services and psychosocial assessments to patients and their families, assisting with various challenges they may face during their healthcare journey.

Responsibilities

• Offer psychosocial assessments to patients and families to address common issues such as adjustment to institutional living, financial concerns, and family relationships.
• Provide counseling and support related to medical diagnoses, aging, discharge planning, grief, and loss.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience as an LBSW, LMSW, or LGPC.
• Strong knowledge of social work theory and practice.
