Job Description
We are looking for a compassionate and skilled PRN Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various challenges such as adjustment difficulties, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and develop care plans.
- Provide counseling and support to individuals and families facing medical challenges.
- Assist with discharge planning and post-care arrangements.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of social work experience.
- LBSW, LMSW, or LGPC certification.
- Strong interpersonal and communication skills.
