Job Description

We are looking for a dedicated Social Worker (PRN) to join our compassionate team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing a range of concerns including adjustment to institutional living, financial issues, family dynamics, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and support patients through end-of-life care.
• Collaborate with multidisciplinary teams to address patients' social and emotional needs.

Requirements

• Bachelor's degree in Social Work or related field.
• Licensed as LBSW, LMSW, or LGPC.
• 1-3 years of social work experience.
