SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients/residents and families.
• Address a range of social and emotional issues including discharge planning and bereavement support.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Requirements

• Minimum of a Bachelor's degree in Social Work.
• 1-3 years of professional experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
