Job Description

We are looking for a compassionate PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families. Your responsibilities will include addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling on end-of-life matters.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
• Collaborate with healthcare teams to develop discharge plans and support patient needs.

Requirements

• Bachelor's degree and 1-3 years of social work experience.
• LBSW/LMSW/LGPC certification.
• Ability to provide compassionate care and support to patients and their families.
