Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges that may arise during medical treatment and institutional living.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist patients with financial concerns and discharge planning.
- Support individuals in coping with aging, medical diagnoses, and end-of-life issues.
- Collaborate with healthcare professionals to ensure comprehensive care for patients.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of experience in social work.
- Licensed as LBSW, LMSW, or LGPC.
- Strong understanding of social work theory and practices.
