Job Description

We are looking for a dedicated and compassionate Social Worker to join our team at LifeBridge Health. As a PRN Social Worker at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a wide range of social and emotional challenges.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address common issues such as adjustment to institutional living, financial concerns, and family relationships.
• Assist with discharge planning and offer support in end-of-life care.
• Collaborate with healthcare teams to ensure holistic patient care.

Requirements

• Bachelor's degree and relevant professional certifications (LBSW/LMSW/LGPC).
• 1-3 years of experience in a clinical social work setting.
• Strong knowledge of social work theory and practice.
• Compassion, empathy, and excellent communication skills.
