SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a member of the hospital's social work team, you will provide clinical services and psychosocial assessments to patients and their families, addressing a range of common challenges they may encounter during their hospital stay.

Responsibilities

• Offer clinical social work services to patients and families dealing with various issues.
• Conduct psychosocial assessments to identify and address patient needs.
• Assist patients with adjustment to institutional living and guide them through discharge planning.
• Provide counseling and support related to end-of-life care and bereavement.

Requirements

• Bachelor's degree and relevant professional knowledge.
• 1-3 years of experience in social work.
• LBSW/LMSW/LGPC certification.
