Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. In this role, you will be providing clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face. Responsibilities

• Provide psychosocial assessments to patients and families.
• Offer counseling on adjustment to institutional living, financial concerns, family relationships, and other common issues.
• Assist with discharge planning and counseling related to death and dying.
• Collaborate with healthcare teams to support patients' well-being.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW/LMSW/LGPC.
• Strong interpersonal and communication skills.
