We are looking for a compassionate SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing a range of common challenges. Responsibilities

• Conduct psychosocial assessments and provide counseling to address various issues.
• Assist individuals and families in coping with medical diagnoses, aging, and end-of-life planning.
• Collaborate with healthcare teams to develop effective care plans and support strategies.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong interpersonal and communication skills.
