SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common issues such as adjustment to institutional living, financial concerns, and discharge planning. Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address various issues affecting patients.
• Assist with discharge planning and counseling related to death and dying.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience in a healthcare setting.
• Licensed as LBSW, LMSW, or LGPC.
• Strong interpersonal and communication skills.
