SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide essential clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address issues related to aging, powerlessness, and end-of-life care.
• Collaborate with interdisciplinary teams to develop comprehensive care plans.
• Assist in discharge planning and post-discharge support.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
