Job Description

We are looking for a skilled SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. As a vital member of our healthcare system, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of common issues including adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Provide counseling on various issues such as family relationships and discharge planning.
• Assist patients in coping with medical diagnoses and procedures.
• Collaborate with healthcare teams to address patients' psychosocial needs.

Requirements

• Bachelor's degree and 1-3 years of related experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of clinical social work theory and practice.
