Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a variety of common challenges related to institutional living, family dynamics, medical understanding, aging, discharge planning, and end-of-life care.
Responsibilities
- Offer clinical social work services and psychosocial assessments to patients and their families.
- Assist with issues such as adjustment to institutional living, financial concerns, family relationships, medical understanding, aging, and end-of-life care.
- Provide counseling and support for patients and families facing difficult situations.
- Collaborate with interdisciplinary teams to develop comprehensive care plans.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
- Strong communication and interpersonal skills.
