Job Description

We are looking for a dedicated Social Worker PRN to join our compassionate team at LifeBridge Health. In this role, you will provide essential clinical social work services and conduct psychosocial assessments for patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address common issues like adjustment to institutional living, financial challenges, and discharge planning.
• Provide counseling on topics related to aging, powerlessness, and death and dying.

Requirements

• Bachelor's degree and professional knowledge in social work.
• 1-3 years of relevant experience in social work.
• LBSW/LMSW/LGPC certification.
