Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. This role involves providing clinical social work services and psychosocial assessments to patients and their families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical procedures and end-of-life issues.

Responsibilities

• Provide clinical social work services to patients/residents and families.
• Conduct psychosocial assessments and address common psycho-social issues.
• Assist with discharge planning and counseling on end-of-life matters.

Requirements

• Bachelor's degree with 1-3 years of experience.
• Licensed as LBSW/LMSW/LGPC.
• Working knowledge of social work theory and practice.
