Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families.

Key Responsibilities

• Offer support for common issues such as adjustment challenges, financial concerns, family dynamics, and end-of-life counseling.
• Assist patients with understanding medical procedures, discharge planning, and coping with changes related to aging.
• Collaborate with the healthcare team to ensure holistic care for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
