SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. In this role, you will provide clinical social work services to patients, residents, and families, addressing various challenges and offering support in times of need.

Responsibilities

• Conduct psychosocial assessments and provide counseling to individuals and families.
• Address issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.
• Collaborate with healthcare professionals to develop discharge plans.
• Offer support and guidance on issues related to aging and illness.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of professional experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Strong understanding of social work theory and practice.
