SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker (PRN) to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various emotional and practical challenges they may face.

Responsibilities

• Offer clinical social work services to patients/residents and families.
• Assess and address common problems such as adjustment issues, financial concerns, and family relationships.
• Provide counseling on medical procedures, aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
