SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services to patients and their families, addressing a variety of common challenges related to institutional living, financial issues, family dynamics, medical procedures, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide support to patients/residents and families.
• Assist in coping with adjustment to medical conditions and treatments.
• Develop discharge plans and facilitate transitions to post-hospital care settings.

Requirements

• Bachelor's degree and relevant professional certification (LBSW/LMSW/LGPC).
• 1-3 years of experience in a similar role.
• Strong knowledge of social work theory and practice.
