Job Description

We are seeking dedicated individuals to join our team as Care Managers at the Agency on Aging of South Central CT. As a Care Manager, you will play a crucial role in assessing the needs of older adults and people with disabilities to provide essential support services. Your responsibilities will include conducting assessments, educating clients about available resources, and ensuring that services are delivered promptly and efficiently.

Responsibilities

  • Complete health, functional, and cognitive assessments of clients.
  • Educate clients on available resources and support services.
  • Assist clients with applications for supportive services and programs.
  • Monitor and evaluate care plans, making necessary adjustments.
  • Ensure all documentation is completed accurately and on time.
  • Meet critical deadlines to minimize service delays.

Requirements

  • Bachelor's degree in health, social work, gerontology, nursing, or related field.
  • Minimum of one year of experience working with older adults or individuals with disabilities.
  • Excellent interpersonal and organizational skills.
  • Familiarity with community resources and state-funded program requirements.
  • Bilingual skills are a plus.
