INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Care Manager, you will collaborate with the clinical team and medical providers to provide discharge planning coordination and intervention for patients with complex health needs.

Responsibilities

• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment.

Requirements

• Bachelor's degree or equivalent professional knowledge.
• BSN required from CCNE accredited schools.
