INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. As an Inpatient Case Manager, you will work closely with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your focus will be on promoting patient wellness, improving care outcomes, and ensuring efficient health services utilization.

Responsibilities

• Collaborate with the clinical team and medical providers to facilitate discharge planning.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients.

Requirements

• Basic professional knowledge; Bachelor's degree preferred.
• BSN required from CCNE accredited schools.
• Prior experience in utilization review or patient care coordination roles preferred.
• Strong communication and interpersonal skills.
