Job Description

As a Primary Care Physician (Bilingual Spanish) at Esperanza Health Centers, you will provide primary medical care services to older adults (>55 years of age) participating in the Program of All-Inclusive Care for the Elderly (PACE). You will lead and train other healthcare professionals, collaborate with the Interdisciplinary Team, and contribute to the planning of health activities within the community.

Primary Duties and Responsibilities

  • Manage primary care practice in PACE Center, participants' homes, and post-acute facilities.
  • Coordinate and develop individualized care plans for PACE participants.
  • Participate in quality improvement activities and ensure sound utilization management of participant care.
  • Communicate with partner facilities to maintain continuity of care and oversight of services.

Requirements

  • Bilingual in English and Spanish.
  • Board certification in primary care required, specialization in geriatrics preferred.
  • At least 1 year of experience working with older adults.
  • Licensed physician in the State of Illinois.
  • Possess valid driver's license and current automobile insurance.
