Job Description

We are looking for a dedicated RN Care Coordinator to join the Ambulatory Care team at Denver Health. As the RN Care Coordinator, you will play a crucial role in facilitating and coordinating care for a diverse group of patients. This position involves patient and family collaboration, assessment, planning, support, and evaluation to ensure quality outcomes within care coordination parameters.

Responsibilities

• Create patient and family centered goals and plans.
• Coordinate care delivery within the clinic and across healthcare settings.

Requirements

• Associate's Degree in Nursing and RN licensure in Colorado.
• Three years of clinical experience in a relevant setting.
• BLS certification.
• Bilingual in English/Spanish preferred.
