Sunrise Senior Living is seeking a compassionate Care Manager to provide top-quality care and services to residents and their families in our assisted living and reminiscence neighborhoods. As a Care Manager, you will build meaningful relationships, assist with daily activities, monitor residents' well-being, and contribute to the development of individualized service plans.

Responsibilities

• Build relationships with seniors and families, provide assistance with daily living activities, and understand individual preferences.
• Notify management of condition changes and recommend care adjustments.
• Participate in developing Individualized Service Plans and ensure compliance with safety regulations.
• Support residents in life skills activities and ensure a warm dining experience.

Qualifications

• Dedication and passion for serving seniors with exceptional customer service.
• Positive attitude, flexibility, and ability to work effectively in a team.
• High School diploma/GED; certifications may be required.
• Prior experience working with seniors is preferred.
• Strong communication, organizational, and decision-making skills.
